Young adult transition

Supporting the Journey Into Adulthood

Moving from school into adulthood can be exciting — but also overwhelming. At Oak Tree Life Skills CIC, we support young people (16+) with autism, learning disabilities, or physical and sensory needs as they navigate this major life change. Our tailored transition support helps build independence, confidence and real-world skills at a pace that works for each individual.

This isn’t a one-size-fits-all service — it’s a flexible, person-centred program designed around real goals, abilities and interests.

We work closely with families, carers, schools, and professionals to ensure the move from education

into adult life is as smooth and empowering as possible.

Here’s what we focus on:

From managing money and preparing meals to using transport and handling personal care routines — we break each step down into achievable goals and repeatable routines.

We help young adults build confidence in group settings, learn how to express themselves clearly, and practise important day-to-day conversations — whether it’s ordering food or introducing themselves in new environments.

Workshops cover digital safety, setting boundaries, understanding emotions, and building healthy relationships. We also support mental and emotional wellbeing through movement, music, and mindfulness-based activities.

As each individual progresses, we explore vocational pathways — including volunteering, community activities, and potential work placements within our centre. This provides a sense of purpose and helps each person contribute meaningfully to their community.

We provide a structured yet welcoming space, free from judgement and full of encouragement. Our team includes professionals from educational, therapeutic, and social care backgrounds — ensuring each young person is supported holistically.

We also maintain low staff-to-service user ratios, allowing time, patience and personalised support every step of the way.
